Photography consent & release form

I (the above signed) do hereby confirm the consent given you with respect to videotaping me/my child in connection with your motion picture/video:

Video Title: Allegro Music School Promotional Content 

I hereby grant you, your successor, assigns, and licensees the perpetual right us use, as you may desire, all video and audio recordings which you make, and the right to use my name or likeness in or in connection with the exhibition, advertising, exploitation or any other use of said recording.*

I also understand that it takes a significant amount of time to complete and film/video. If the filmmaker has promised a tape of the video I agree to allow a reasonable amount of time to elapse after the performance for completion (six months).

I agree that should the film/video not be completed I will take no action against Liberty University, the students or Allegro Music School. *

I am over 18 years of age
